This is the UD-082 Double Door Left, part of the Dungeon Blocks modular terrain range. The sculpt depicts a heavy wooden door, reinforced with iron studs, draped in a heavy chain, and mounted with a skull centrepiece that makes its purpose unmistakably clear: this is not a door you open without consequence.
