This is the Toxic Sewer Ground Poison Trap (TS-023) from the Dungeon Blocks Toxic Sewer terrain range. The sculpt depicts a raised stone tile overtaken by writhing green tendrils bursting through the surface — the kind of environmental hazard that makes a Dungeon Master grin and a rogue immediately start looking for a disarm mechanism.
