The Neh-Thalggu — sometimes called the Brain Collector — is one of the most unsettling aberrations to haunt the Underdark and the Far Realm's fringes. This sculpt by Monster Atlas captures every grim detail: the swollen, tumour-ridged body bristling with worm-like tendrils at the crown, a cluster of bulbous eyes set above a wide, tooth-ringed maw, and eight long segmented legs each ending in hooked claws.
