This is Tomb 19 from the Haunted Graveyard terrain range by Dungeon Blocks, reference HG-053. It depicts a stout, tiered tomb plinth topped with a carved stone monument bearing a circular religious motif — the kind of weathered marker that suggests someone important, or someone dangerous, lies beneath. The base of the monument is heaped with sculpted skulls, spilling across the stonework as though the earth itself has been slowly surrendering its dead.
